Every year, all health plans in New York State are required to collect and submit a wide range of data to the Department of Health. The plans report on who their providers are and how they are paid; what kinds of services their members receive; how many members are receiving certain preventative services and other indicators of good care; and how satisfied members are with the health plan's quality of care and service (the Health Accountability Foundation's HMO Report Card is based on this QARR data).
For HMO care, the site contains HMO Performance Reports for calendar year 2005. These reports give each New Jersey HMO's performance on nationally-used measures of health care quality and service, and compares the performance to state-wide and national averages.
The report contains information for each MCO on the number of its providers, its performance on quality-of-care measures, on ease of access to medical services, and on member satisfaction with care and services.
Consumer Guide offers descriptions of health insurance products available in New York State, information on how to choose a health plan and easy-to-read tables detailing comparisons between health insurers in terms of their quality. The Guide explains how consumer complaints, grievances and internal and external appeals are handled. A listing of the names and telephone numbers for all insurers included in the Consumer Guide is provided.

The information in this report comes from data that all HMOs operating in the state of New York are required to submit annually to the New York State Department of Health.
